Community Health Needs Assessment
Grand Itasca's Community Health Needs Assessment process is valuable work that helps us address our mission of "Patient-Centered Excellence."
Community Health Needs Assessments are detailed reports that provide insight into the health needs of the communities we serve. By conducting these assessments on an ongoing basis it allows us to better meet the needs of our community.

Patient and Family Advisory Council
At Grand Itasca, we know that patients and their families have important insights that can help us improve our care. To support this, we created the Patient & Family Advisory Council, a collaborative group bringing together diverse perspectives to make a meaningful difference.
This council gives you the chance to share your experiences, ideas, and feedback to help shape the care we provide and make it better for everyone.
Who Can Join?
We invite current patients, family members, and caregivers to become a patient-family advisor. By sharingyour stories and perspectives, you’ll contribute to a care model focused on the needs of patients and families, leading to enhanced experiences for all.
We want the council to reflect the diversity of our patients and families, so everyone feels heard. The group has up to 18 members, with most seats reserved for patients and family members. Members commit to at least one year and can serve longer if they'd like.

Joining is a great way to give back and make a real impact. Together, we can ensure that every individual at Grand Itasca feels respected, supported and cared for.
