Land Acknowledgment

Fairview’s systemwide Land Acknowledgment inspired us to explore a more localized approach that reflects the unique history and Tribal Nations connected to our region. Our team at Grand Itasca engaged employees, community partners, and local tribal members to thoughtfully create a Land Acknowledgment for our site. Along the way, we recognized the importance of building stronger relationships and deepening our understanding, and we have continued that work with intention. We are proud to share this Land Acknowledgment to honor both the land we occupy and the Indigenous peoples who have stewarded it.

Grand Itasca Clinic & Hospital Indigenous Land Acknowledgment

We gratefully and respectfully acknowledge Grand Itasca Clinic & Hospital is located on the traditional, ancestral, and contemporary lands cared for and called home by Indigenous people.

This sacred land does not belong to us. We are occupiers here who have also come to call this land home, where the water reflects the sky.

Mni Sota Makoce (Minnesota) is the homeland of the Dakota and Anishinaabe peoples and other Native nations, whose ancient relationships and historic connection to this land continue to this day.

We recognize and honor the eleven tribes that call Minnesota home - including our closest Tribal nations of Leech Lake, Fond du Lac, and Bois Forte – and are committed to ongoing efforts to recognize, support and advocate for Native nations and peoples.

We acknowledge the broken treaties of 1854 and 1855 and the complex and layered history of this land, which holds great historical, spiritual, and personal significance for its original stewards, the Native nations, and peoples of this region.

While we cannot undo the wrongs and do not want to disguise the past, we must be forthright about the journey today and thus take restorative action.

We realize a land acknowledgment is only a starting point. We must understand the impacts of this history on the generations of the past and the generations of the future and our ongoing relationships, action, and support for Indigenous communities.

By extending and fostering friendships and partnerships with our Indigenous neighbors, we seek to pursue a greater level of awareness, respect, appreciation, and understanding for our place in history. We endeavor to learn and lead by example through our words, actions, and reverence for this sacred land on which we live, work, and serve.

Community Health Needs Assessment

Grand Itasca's Community Health Needs Assessment process is valuable work that helps us address our mission of "Patient-Centered Excellence."

Community Health Needs Assessments are detailed reports that provide insight into the health needs of the communities we serve. By conducting these assessments on an ongoing basis it allows us to better meet the needs of our community.

To request a paper copy of any Community Health Needs Assessment or for questions/comments please email Kelly Campbell.

Patient and Family Advisory Council

PFAC logoAt Grand Itasca, we know that patients and their families have important insights that can help us improve our care. To support this, we created the Patient & Family Advisory Council, a collaborative group bringing together diverse perspectives to make a meaningful difference.

This council gives you the chance to share your experiences, ideas, and feedback to help shape the care we provide and make it better for everyone.

Who Can Join?

We invite current patients, family members, and caregivers to become a patient-family advisor. By sharingyour stories and perspectives, you’ll contribute to a care model focused on the needs of patients and families, leading to enhanced experiences for all.

We want the council to reflect the diversity of our patients and families, so everyone feels heard. The group has up to 18 members, with most seats reserved for patients and family members. Members commit to at least one year and can serve longer if they'd like.

How to Get Involved
